Relocate template: '...' properties to templateUrl: '...' and extract the HTML

Relocate template: '...' properties to external files and use templateUrl: 'select.html' (approximately) instead.

We can then create a select2 version of select.html and a bootstrap version, etc.

Example path:

src/themes/select2/select2.css
src/themes/select2/select2.png
src/themes/select2/select.html // or whatever

Since you're aiming for just getting things working, just hard-code the path to the select2 version and we can improve this later.
