Add package manager + module loader examples for all plugins

We only show you how to load plugins via CDN. We need examples for other loading mechanisms.

I could contribute some docs for requirejs+angular later, and for loading via <script> tags (#476) if there's a tagged beta build (?). I assume that set up and config for the other plugins will be largely the same?

@geotob – you're totally welcome to, but yeah, I'd probably wait for 2.1.0 to land / go live. I'm also perfectly content doing the docs myself (it is my job, after all), in which case I'd be awfully glad to have you as a reviewer. Your call.

Sounds good @benvinegar , happy to review when you push 2.1.0. Depending on how long you're planning to have 2.1.0 in beta it may be good to put a warning in the docs... something like:

To use any of the plugins, we highly recommend using the pre-built source from our CDN (www.ravenjs.com). Support for local hosting will be added in the upcoming 2.1 release.
